Housing Support

Are you looking for housing? Avivo is not able to self-refer to our own housing (such as Avivo Village and other supportive housing options), so all referals must come via Hennepin County’s Coordinated Entry System. Coordinated Entry is a service to get the process started for moving into transitional housing/shelter and they’d assign you to shelter. Visit https://www.hennepin.us/en/residents/human-services/homelessness-resources if you live in Hennepin County and call their Shelter Hotline, 612-204-8200, OR call United Way’s 2-1-1 (dial 211 on phone). If you’re working with a street outreach worker, they can also add you to the list.

Visit our Ending Homelessness services page to learn more about housing supports offered at Avivo and links to obtaining housing if you’re in immediate need. Coordinated Entry exists in every county in Minnesota, so if individuals seeking shelter should contact the County office in their county of residence.

Single adults, youth, and families in St. Paul can visit Ramsey County Housing Services & Support for more information.

The Youth Services Network helps young people (24 years old and younger) to find a safe, temporary place to stay. See shelter availability for youth shelters in the metro area at ysnmn.org/shelters.

Community Support Program

Avivo’s Community Support Program (CSP) supports adults in Hennepin County living with a severe and persistant mental illness.
