Chemical Health Assessments FAQ

 

Can I come before 7 a.m.?

Doors open at 7:00 a.m. April through October, and at 6:30 a.m. November through March. Many people arrive and start lining up in front of the building as early as 6:30 a.m.

 

Can I bring my children?

Please make every effort to make arrangements for your children so you can come on your own.

 

Can I bring a support person with me?

Yes, you may bring a supportive adult.

 

Which treatment providers will be available to me?

Your assessment will be valid for any treatment provider based on your funding and insurance. Your assessor will recommend a treatment provider based on your situation and needs.

 

Is there a cost?

There is no cost if you meet eligibility guidelines. For most individuals, the fee is covered by insurance, PMAP, and CCDTF. If you do not meet eligibility guidelines, the self-pay fee is $125.

 

Can the assessor give me a mental health diagnosis?

Your assessor cannot make a mental health diagnosis, but may suggest that you will benefit from receiving a mental health assessment. If you do receive a mental health diagnosis, you may be considered for our dual-diagnosis treatment program.

 

How will you protect my privacy?

Avivo complies with all Minnesota Data Practices Act and HIPAA regulations. We need to get a signed release of information from you before we would share any information. If we are asked whether someone is receiving services, we respond that we are not able to share this information.
