Case Management

Adults living with mental illness

We offer case management services, short-term and long-term, to help adults with severe and persistent mental illness (SPMI) live successfully and independently. Our case managers provide support for individuals who are at the greatest risks and need life-changing assistance. Individuals are connected to care coordination, housing support, coordinating of physical and health supports, and intensive goal planning and management.

Depending on a person’s needs, case management includes:

  • Assessment
  • Case planning
  • Advocacy
  • Care coordination
  • Housing assistance or coordination
  • Monitoring and support
  • Crisis evaluation and assistance
  • Connection to community wellness and social activities

Costs for services are covered by insurance.

Avivo currently provides long-term and short-term case management for recipients in Hennepin county, and offers short-term case management services additionally for clients in Anoka and Dakota counties.

Mental health case management services for families receiving MFIP

Our HealthChoices program coordinates chemical and mental health services for individuals and their children who are currently MFIP (Minnesota Family Investment Partnership) benefits. Adults and/or their minor children may receive psychiatric services, therapy, learning disability assessments, and connection to medical and dental care. Services help stabilize families and support individuals’ employment goals.

MFIP counselors are encouraged to refer participants or learn more, call 612.752.8181.

Referring professionals may download a referral form and fax it to 612.824.4654.
