Getting an assessment

**Walk-in assessments are temporarily paused at our 1900 Chicago Avenue location until August 10.**

**We are at capacity for virtual assessments this week, but walk-in assessments will be restarted Monday, August 10.**

Starting August 10, in-person assessments will resume on a first-come first-served, walk-in basis, Monday – Friday, at our Minneapolis location at 1900 Chicago Avenue. Doors will open at 7:00 a.m.

Entering Avivo’s treatment programs

To enter Avivo’s treatment programs, individuals must have a chemical health diagnosis which is documented by a Chemical Health Assessment. Individuals may have no insurance or be on a PMAP insurance plan (no private insurance is accepted). The chemical health assessment provides access and funding for substance abuse treatment.

Admittance to our residential and outpatient treatments is paused. Scheduling intakes will resume starting until August 10. To schedule or refer, email intakereferrals@avivomn.org, call 612.752.8074, or fax 612.752.8051.
